Capsule Endoscopy (CapsoCam) Patient Education & Preparation

What Is This Test?

A CapsoCam is a capsule endoscopy study that uses a vitamin-sized camera to take thousands of pictures of your digestive tract as it naturally travels through your system, helping your provider evaluate symptoms such as unexplained abdominal pain, bleeding, anemia, Crohn’s disease, inflammation, or other small bowel concerns.

What to Expect

At your appointment, you will swallow the capsule with water; its tiny cameras capture images throughout the day as it moves through your intestines. You may resume most normal activities unless instructed otherwise, and the capsule will pass naturally in your stool.

***IMPORTANT PREPARATION INSTRUCTIONS***

PLEASE NOTE: Not following these instructions can prevent us from safely or accurately completing your test. Please follow all preparation steps carefully to avoid delays or rescheduling.

Several Days Prior to Exam

  • ***IMPORTANT: Stop all oral iron supplements 5 days before your procedure***
  • Review all medication instructions from your provider and notify us if you have trouble swallowing, a history of bowel obstruction, prior abdominal surgeries, or severe constipation.
  • Purchase and review your bowel prep ahead of time (you will need at least 153g of polyethylene glycol and 1.2 mL of Mylicon Drops).

Day Before Your Procedure

  • Eat a normal breakfast and a light lunch, then begin a clear liquid diet for the rest of the day (NO red-colored liquids).
  • At 7:00 PM, mix seven (7) capfuls (119 g) of polyethylene glycol with 32 oz of water, Gatorade, or sugar-free Gatorade (non-red) and drink all contents.
  • At 9:30 PM, take 0.6 mL of Mylicon drops with 8 oz of water to reduce bubbles in the GI tract.
  • Do not drink anything after 10:00 PM, except small sips of water with medications.

Day of Your Procedure

  • Hold all medications until 2 hours after capsule ingestion unless otherwise instructed; then take with small sips of water.
  • Two (2) hours before your appointment, mix one (1) capful (17 g) of polyethylene glycol with a full glass of water or non-red Gatorade and drink all contents.
  • Swallow the CapsoCam Plus capsule with 0.6 mL of Mylicon drops in 8 oz of water.
  • Resume clear liquids 2 hours after swallowing the capsule.
  • Four (4) hours after ingestion, drink another 17 g dose of polyethylene glycol mixed with a full glass of water or (non-red) Gatorade, then you may have a light lunch.
  • You may eat normally for dinner; no further diet restrictions are needed.
  • Avoid MRI studies until the capsule has passed.

After the Procedure

  • The capsule should pass naturally in your stool within 3–30 hours; continue using the retrieval pan until it is recovered

  • Once retrieved place the capsule in the vial, secure the lid, seal it in the pre-labeled padded envelope, and return it via any FedEx drop-off or drop box.

***Call the office if you develop abdominal pain, nausea, or vomiting, or if the capsule has not passed within 72 hours.***

Upper Endoscopy (EGD) Prep

An EGD (upper endoscopy) is a simple procedure that allows your doctor to look at your throat, stomach, and the first part of your small intestine using a small camera. Following these instructions carefully is very important. Reading all instructions keeps you safe and allows your doctor to see clearly and provide the best care possible.

FOUR DAYS BEFORE PROCEDURE:
Discontinue multivitamins as well as iron, fiber, herbal and homeopathic supplements (unless advised by a physician).

ONE DAY BEFORE PROCEDURE:
If you are taking GLP-1 medication (such as Ozempic, Wegovy, Mounjaro, Zepbound, Trulicity, or similar), you must follow a clear liquid diet the entire day before your procedure.

DAY OF PROCEDURE:
Clear liquids only, NO SOLID FOOD. Not following day of procedure instructions may cause your procedure to be cancelled due to safety concerns with anesthesia.

6 HOURS BEFORE PROCEDURE:

  • Stop all forms of tobacco products INCLUDING ORAL CHEWING TOBACCO.

3 HOURS BEFORE PROCEDURE:

  • Nothing by mouth at this time.
  • If you have anything by mouth, your procedure may need to be canceled. This includes clear liquids, gum, chewing tobacco, and hard candy

Transportation

If you are having sedation, you must have a driver who will stay at the endoscopy center during your procedure and drive you home. No driver, no procedure.

A taxicab is allowed only if a responsible adult accompanies you and will go home with you.

Medication Instructions

At least 3 hours before your procedure take inhalers and medications for heart, blood pressure, seizures, anxiety, and breathing with a small sip of water

DO NOT take blood thinners or diabetic medications unless instructed.

Blood thinners:

Blood thinners include prescription medications, aspirin (including baby aspirin), injections, and some supplements. If you take any medication to prevent blood clots or thin your blood, please notify our office.

We will coordinate with your prescribing provider and let you know when (and if) you need to stop any medication. Please do not stop medications on your own. If you haven’t heard from us, please call us!

  • Warfarin (Coumadin, Jantoven)
  • Cilostazol (Pletal)
  • Dipyridamole (Persantine Rivaroxaban (Xarelto)
  • Apixaban (Eliquis Diabetic medications:
  • Dabigatran (Pradaxa Edoxaban (Savaysa)
  • Clopidogrel (Plavix)
  • Ticagrelor (Brilinta)
  • Prasugrel (Effient)

Diabetic medications:

  • Please check with your prescribing Physician or Endocrinologist on how to adjust your insulin.
  • Do not take your diabetes medications the evening before and day of your procedure – but bring them with you.
  • Please check your blood sugar morning of procedure.
  • Procedures will be cancelled if blood sugar is 400 or above.
  • If you are concerned about low blood sugar, consider purchasing glucose tablets.

Diet medications:

  • Hold 7 days before your procedure (Phentermine, Adipex, Qsymia
  • If on a GLP1 medication, you must follow a clear-liquid diet the day before the procedure. It is not necessary to hold these medications.

Upper Endoscopy (EGD) and Sigmoidoscopy Prep

If you normally take Pletal (Cilostazol), Jantoven, Plavix (Clopidogrel), Coumadin, Warfarin, Persantine (Dipyridamole), Eliquis (Apixaban), Aggrenox, Xarelto or Pradaxa (Dabigatran) then we will contact your prescribing physician to make sure that it is okay for you to stop for the recommended number of days. We will contact you once they have responded. Do not stop aspirin for the procedure.

FOUR DAYS BEFORE PROCEDURE: Discontinue multivitamins as well as iron, herbal and homeopathic supplements (unless advised by a physician).

ONE DAY BEFORE PROCEDURE: Normal diet until midnight. After midnight, clear liquids only such as broth or bouillon, Jell-O, popsicles and Gatorade (NO red or purple), tea, coffee, clear soft drinks and water.

NO SOLID FOOD OR MILK PRODUCTS.

DAY OF PROCEDURE:

You may have clear liquids, gum, hard candy and tobacco products up to 3 hours before your procedure.

  • Use one Fleet enema (directions are on the box) two (2) hours before you plan on leaving home.
  • Use a second Fleet enema one (1) hour before leaving home.
  • If you are traveling a long distance, begin the enemas sooner than 2 hours and one hour before leaving home.

* IF YOU TAKE BLOOD PRESSURE MEDICATIONS, YOU MAY TAKE THEM 3 HOURS PRIOR TO YOUR PROCEDURE TIME.

*IF YOU ARE A DIABETIC, DO NOT TAKE YOUR MEDICATIONS BUT PLEASE BRING THEM WITH YOU.

*DO NOT TAKE HEPARIN OR LOVENOX ON THE DAY OF PROCEDURE.
